Investigation Summary
On March 24, 2007, Employee #1, a sanitation employee, was cleaning the inside of a screen housing unit. The unit was turned on at the breaker box, which resulted in the machine being turning on. Employee #1's left arm was amputated by the rotating paddles. Employee #1 was hospitalized.
