Investigation Summary
On January 29, 2007, Employees #1 and #2 were on their knees, while performing maintenance on a Styrofoam mold. Another employee operating a forklift bumped an adjacent mold, causing it to rock and then fall on top of the two kneeling employees. Both employees were hospitalized with unspecified injuries. However, Employee #1 died on January 30.
