Employee Is Killed in Fall

Employee #1 was on site for an evaluation in preparation for bidding for a window-washing contract. Employee #1 was operating an Up-Right, Inc., (Model Number 62696-002) 38-ft aerial lift platform with only one outrigger installed. (Four outriggers are required by the operator's manual.) Employee #1 initially elevated to approximately 12-ft above pavement to test wash a window. Employee #1 finished that window and elevated to another, and test washed it as well. Employee #1 then continued to the upper window elevation at approximately 37-ft and the aerial lift tipped over for unknown reasons. Employee #1 remained inside the platform of the lift and impacted a concrete curb and grassy area sustaining fatal internal injuries. Employee #1 was killed.
