Employee's Arm Is Injured While Reaching Into Machine

At approximately 3:30 a.m. on June 15, 2009, Employee #1, a maintenance worker with Pacific American Fish Company, Inc., was performing a preproduction service check of a machine known as "the Portioner". This machine electronically scans whole fish filets and slices them into the desired size that is programmed into the onboard computer by the machine operator. Within the machine is knife blade that rotates clockwise in wheel type track. When Employee #1 observed that the conveyor belt was not advancing smoothly and that the conveyor motor was wobbling, he reached into the machine without deenergizing it. The knife blade lacerated Employee #1's left forearm. He was hospitalized at USC County Medical Center for treatment, including surgery. Employee #1 stated that he was not trained in the hazards of the Portioner machine. The Employer did have in place a specific lockout/tagout program for the machine.
