Investigation Summary
At approximately 8:15 p.m. on April 1, 2009, Employee #1 was operating a machine that manufactures ice cream containers. Employee #1 noticed that the machine next to hers was about to run out of paper for the bottom board or paper to make the base of the ice cream cup container. Employee #1 said that this was standard procedure for the company, to take the initiative to help each other out. Employee #1 stated she followed company procedures to reload the machines. Employee #1 restarted the machine but the paper jammed. Employee #1's thumb was caught or jammed in the area of the punch and die and the mandrel of the machine during an attempt to unjam the paper. Employee #1 was hospitalized and underwent surgery. Employee #1's thumb had to be amputated. Employee #1 was released from the hospital on April 3, 2009.
