Employee Amputates Finger While Moving Case with Dolly

On the day of the accident, Employee #1 was assigned by his immediate supervisor to load a truck with furniture for delivery. Employee #1 and a coworker were placing a glass display case on top of a 4-wheel flat red dolly to load it on a delivery truck. The coworker lifted the glass display case from one side, and Employee #1 placed a 4-wheel flat red dolly underneath. The wheels were perpendicular to the display case. As the glass display case was released by the coworker, the dolly slipped, and the wheels of the dolly became parallel to the display case. As a result, the dolly fit inside the hollow space, causing the display case to hit the floor. The 4-wheel flat red dolly was not of an adequate size to safely support the display case and the display case was not adequately secured to prevent displacement of the dolly. Employee #1's third finger of his left hand was crushed and partially amputated. Employee #1 was treated for his injury but was not hospitalized.
