Investigation Summary
On August 5, 2008, Employee #1 was standing on the top plate of the fourth floor when he inadvertently lost his balance and fell to the third floor. The distance between the third and the fourth floor was about 12 ft. Employee #1 fractured his foot. He was initially taken to a clinic. On August 18, 2008, Employee #1 was transported to Harbor UCLA Medical Center in Carson, where he was hospitalized by August 25, 2008. During the interview, Employee #1 was unable to provide details regarding the incident.
