Investigation Summary
At approximately 4:45 p.m. on July 22, 2008, Employee #1 was an order collector at the 99-Cents Only Store in Commerce, California. He was using a pallet jack to drop off a load. After dropping the load, he was riding on the front of the empty pallet jack traveling approximately 10-15 miles per hour east bound at location 70 Pick Bin 31. This bin was located on a 10-ft, 2-in. aisle. He maneuvered the pallet jack to the right towards an approximately 2-ft high pallet of metal racks. His right foot struck a metal rack member piece that was projecting outward approximately 1-ft. The employee sustained an open fracture and laceration of the right foot. He was transported to County USC Medical Hospital. Employee #1 was hospitalized and underwent surgery.
