Employee Trips on Shower Drain, Injures Head

At approximately 11:50 a.m. on July 2, 2008, Employee #1, a project manager with the Los Angeles Community College District, was on site at East Los Angeles Community College located at 1301 Avenida Cesar Chavez, Monterey Park, CA. He was inspecting new construction work on a fitness center (Building C-1) of the facility. In the future shower room, there was a floor drain located in the approximate center of the floor; the drain measured five inches in diameter and protruded approximately two inches above the floor. While walking in the area, Employee #1 tripped on the drain and fell to the concrete floor, striking his head. According to witnesses there were two light stands in the area and Employee #1 stated that he did not have trouble seeing.
