Employee's Finger Is Amputated When Pinched

On June 1, 2008, Employee #1 was a driver working for a stage scenery prop supply company. He was guiding a large table being loaded onto his truck via a forklift truck. Employee #1's finger became caught in a pinch point between the table and another object that had been previously loaded. Employee #1 was hospitalized for an amputated finger.
