Investigation Summary
At approximately 12:17 a.m. on April 8, 2008, Employee #1 was working as an assembly worker for a firm that produced frozen foods. Frozen food entree plates were produced at the facility where she worked, which had 1,100 employees. She had been working there since December 4, 2006, performing the duties of an assembly worker, including ensuring that packaging equipment was working properly. At the time of the accident, she was performing a regularly assigned task of ensuring that a packaging machine was working properly. The machine was an FEMC Model Number 1192 655 heat sealer, with Serial Number 6982. It had stopped due to a jam of packaging film in the feed section. Employee #1 was adjusting the packaging film that had bunched up during a setup procedure. She reached her right hand under the machine guard and into the film carriers, without deenergizing the machine, to adjust the plastic film that had bunched up. The machine began a production cycle, crushing the tip of the middle finger on her right hand. She was transported to a local clinic for pain management and then to a local hand surgery center, the Western Hand Center, where the fingertip was amputated. There was also some bone loss. Following surgery, she was released.
