Investigation Summary
At 9:00 a.m. on December 18, 2007, Employee #1 was working on Palletizer Number 2, filling in for a coworker who was off work. Employee #1 was doing a set up to change out the bag size. Employee #1 waited for the bags to start coming through the palletizer, but said the bags were taking too long to come through. Employee #1 decided to go over to the area where the bags come from to see if another coworker, who is responsible for loading the bags, needed help since the coworker was a new employee. When Employee #1 reached his coworker, the coworker had already sent some bags through. Employee #1 noticed that eight bags were sent instead of five. Employee #1 went back to the palletizer and turned off the motion and in-feed mode on the PLC computer. Employee #1 then climbed into the machine to remove the extra bags. Once the bags were removed the palletizer cycled, trapping Employee #1's right leg. Employee #1 sustained a seriously fractured right leg. Employee #1 was taken to Montebello Beverly Hospital where he was treated for his injury.
