Investigation Summary
On December 4, 2007, Employee #1, an experienced journeyman roofer, was working at an affordable housing project consisting of 33 production-type residential dwellings. The roofs had pitches ranging from 3-in-12 to 9-in-12. Employee #1 had been the only one working on the roof that day and, at the time of the accident, had completed his work hours and was preparing to come down. He was walking on the roof top, right above the front porches on the west side of Building B, when he apparently lost his balance and fell aproximately 19.5 ft to the dirt surface at ground level. He sustained injuries and was transported to Huntington Memorial Hospital, where he was admitted for treatment. Subsequent investigation determined that the accident was caused by Employee #1 prematurely disengaging his lanyard from the anchorage before he had safely reached ground level.
