Employee Is Struck in the Face by Flying Object

At approximately 10:17 a.m. on December 4, 2007, Employee #1 was in the grinding and polishing department polishing an aluminum work leg from a piece of furniture. He was standing in front of a buffing machine, Model Number BA 00104 TF 208-230/460-volts 1740 rpm, Serial Number WNA-4 02549 AA, Electric Motors Hayward 94565 3 phase 60-Hz, Buffing wheel 14-in. wide diameter, holding the work against the buffing wheel. The buffing wheel grabbed the work piece and it slipped from his hand and struck him in the face. He was hospitalized with fractured nasal bones, multiple lacerations, abrasions and dental injuries.
