Employee Is Injured in Forklift Accident

On October 30, 2007, Employee #1 had been picking orders and drove his forklift to the warehouse to pick up an order. While standing next to his forklift to scan his order, Employee #2 came towards him. Employee #2 apparently lost control of his forklift and struck Employee #1's forklift, causing it to wobble and come down on Employee #1's right toe. Employee #1 sustained a serious injury of a crushed right toe. Employee #1 was taken to Gardena Memorial Hospital where he was treated for his injuries and was hospitalized for five days. When he arrived home, his father was concerned about the foul odor coming from his toe and took Employee #1 back to the doctor. The doctor sent Employee #1 to a hospital in Hawaiian Gardens where they amputated his right toe.
