Investigation Summary
At approximately 12:30 p.m. on September 9, 2008, an employee was working in a warehouse. She was run over by a CAT forklift truck operated by a coworker when he backed into her at the loading deck area in between Door Number 46 and Door Number 48. She was still breathing at the scene when the paramedic arrived and was transported to St. Francis Hospital in Lynwood where she died.
