Employee Is Struck and Killed by a Loader

At approximately 11:45 a.m. on January 24, 2007, Employee #1 was marking the ground with lime in area 4.2 to identify the locations for foundations. Sometime before 11:45 AM, Employee #1 left area 4.2 to obtain additional lime located in a storage area located west of area 4.2. At approximately 11:45 a.m. Employee was walking eastbound, through area 5.2, with a bag of lime, in the direction of area 4.2. While Employee #1 was in area 5.2 he was ran over by a loaded 5-cubic-yard, articulated, wheeled, front loader, Kawasaki Model Number 90ZV, Serial Number 90C5- 5023, being driven in reverse, and making a right turn, by another employee. Employee #1 sustained massive upper torso trauma, after being ran over by the left front tire of the loader, resulting in his death.
