Employee Injures Foot When Run over by Truck

On December 12, 2006, Employee #1 was working for a company that provides warehousing and trucking services. Employee #1 was performing inventory of an aisle in a customer's warehouse when a coworker, who was operating a Crown stand-up style reach truck, began talking with her. As Employee #1 and the coworker were talking, the coworker unintentionally moved the joy stick of the truck with her right elbow. This caused the reach truck to move backward and ran over Employee #1's right foot. Employee #1 was transported to Harbor UCLA Medical Center in Torrance, where she was hospitalized and treated for her injury. The accident investigation determined that the accident was caused by the truck operator attempting to approach Employee #1 while operating the reach truck. Additionally, the truck operator failed to deenergize the machine when it was parked. The employer was cited for not enforcing the proper operating rules to ensure the safety of employees working in the warehouse area.
