Investigation Summary

Investigation Nr: 202453353
Event: 12/12/2006
Employee's Fingers Amputated in Chain and Sprocket Drive

At approximately 3:40 p.m. on December 12, 2006, Employee #1, a personal computer technician, was working for an employer that repaired personal computers. Employee #1 was working alone on a conveyor line in the bench repair area removing computer circuit boards from a conveyor. The conveyor was powered by a 220 V .25 HP motor and consisted of two sets of drive chains equipped with wheels along the entire length on which materials were placed. The circuit boards were resting on particle board caddies that were moving along the conveyor. Employee #1 was wearing a pair of slip resistant gloves, when he grabbed the caddies at the hand hold cutouts. While removing the circuit boards on a caddy, Employee #1's right glove became entangled in the chain and sprocket drive of the conveyor pulling his hand into the machine, amputating his fingers. Employee #1 was taken to the hospital, where he was treated for his injuries.
