Investigation Summary
At approximately 6:15 a.m. on November 22, 2006, Employee #1 was a leadman maintenance worker with Imperial Shade and Venetian Blind. He was preparing to repair a damaged electrical outlet. To de-energize the outlet, Employee #1 was visually tracing the conduit back to the disconnect panel. He was using a Louisville metal and fiberglass extension ladder to view the conduit at the ceiling. The ladder was leaning against a structural member. After he had climbed to the eleventh or twelfth rung, the foot of the ladder slipped. Employee #1 briefly hung from the structural member, but was injured when he fell to the floor. He was hospitalized for unspecified fractures.
