Investigation Summary
At approximately 9:46 p.m. on December 26, 2007, Employee #1 was found in the prone position beneath the right rear dual tires of an Ottawa Industrial Tow Tractor by another employee, who was attempting to back in an empty semi-trailer for parking in the yard. The truck tires were resting on Employee #1's back. According to first responders, the vehicle was found to be in reverse with the tractor air brakes engaged. The weather the night of the incident was cold, with approximately two to three inches of accumulated snowfall on the ground and patchy areas of ice in the yard underneath the snow and the vehicle's rear tires.
