Employee's Finger Is Amputated by Forklift

On October 2, 2007, Employee #1 and Employee #2 were preparing a forklift for use in the unloading of rolls of irrigation tubing. The distance between two forklifts needed to be wider so they first moved one fork outwards. For the second fork, Employee #2 was on the end of the forklift (on the outside), while Employee #1 was kneeling next to the fork (on the outside), closest to the body of the forklift. Employee #1 positioned his left hand around the top part of the vertical member of the second fork and his right hand around the lower part of the vertical member. As the two employees pulled the fork towards themselves, the fork slipped out of Employee #2's hand. Because it was a hinged type fork, it came down so that the vertical portion struck against a horizontal member in back of the fork. Employee #1's right ring finger was smashed, when the fork struck the metal cross piece. A supervisor transported Employee #1 to Lodi Memorial Hospital, where he was treated and released the same day. Employee #1 sustained an amputation to about 0.25 in. of his right ring finger.
