Employee injured in fall descending ladder into enclosure

On October 4, 2007, Employee #1 was laying pipe inside a 10 ft by 10 ft by 24 ft deep metal enclosure. Inside the metal enclosure, he went down a ladder set between the concrete side of an 8 ft by 8 ft by 24 ft deep precast concrete manhole and a 10 in. thick metal shield covering the manhole to prevent dirt from entering it. He was descending the ladder when he stepped on one of the rungs and lost his footing. Employee #1 fell, head first, approximately 21 ft to the bottom of the enclosure. He sustained injuries that required hospitalization.
