Investigation Summary
At approximately 9:30 a.m. on March 12, 2008, Employee #1 was delivering a load of empty olive cans to Bell Carter Olives. He had opened the trailer doors and was removing the pins from the pallet rollers for unloading. A forklift operator bumped an adjacent stack of palletized cans, causing them to fall out of the truck and strike Employee #1. He was hospitalized for treatment of unspecified fractures. The forklift operator had been properly trained and certified.
