Employee's fingers injured by falling shaft

On July 24, 2007, Employee #1 and two coworkers were installing a test well as part of a new water well system for the City of Sacramento, CA. As part of the test well assembly, a truck-mounted crane was being used to insert sections of narrow shafts into the well casing. The fourth shaft was suspended over the casing and was secured by an internally made clamp when Employee #1 noticed dirt around where the two shafts would be joined, and he decided to use a rag to clean the area. Employee #1 was holding the cloth in his right hand and wiping the area when the suspended shaft slipped through the clamp and fell, contacting the well casing and catching his hand. Employee #1's coworkers drove him to Mercy General Hospital in Sacramento, where he was treated for a laceration to the tip of his index finger, and the tip of his thumb was surgically amputated. He was released from the hospital the same day.
