Investigation Summary

Investigation Nr: 202485983
Event: 07/10/2007
Employee's Foot Is Amputated When Run over by Crane

On July 10, 2007, an employee with Clark Pacific of West Sacramento was acting as a ground man for a gantry crane operator as they removed precast forms from a production line and placed the forms onto flat-bed trailers. After lifting one form out of the casting bed, the crane operator moved it horizontal until it aligned with prestaged trailers. The crane operator then had to move the crane forward so the form could be lowered to its assigned trailer. As the crane moved, the employee who was on the left side of the casting bed between the trailer and outer frame of the crane was struck by one of the crane's tires. The crane operator saw the employee go down and stopped the crane but in doing so, the tire came to rest on his left foot. The crane and form weighed a total of 100,000 lbs. The West Sacramento Fire Department responded and arranged transportation of the employee to UC Davis Medical Center in Sacramento. The crushing injury to his foot forced doctors to surgically amputate the foot, and he was released on July 18, 2007.
