Employee's Fingers Are Amputated When Caught in Belt Pulley

At approximately 2:50 p.m. on February 12, 2007, an employee and a coworker were testing the Servo control on the Lucas Number 3 Horizontal Boring machine. According to all accounts the Lucas Number 3 was locked and tagged out during the initial installation of the Servo motor. Following the installation of the motor the Lucas Number 3 was energized to test the repair. Immediately, a servo motor overload alarm displayed and the employee and the coworker returned to a troubleshooting mode. The coworker was tracing and checking wire connections within the control cabinet located on the side of the servo motor. The employee was testing the Servo motor by reaching in to the housing compartment of the motor pulley, drive belt, and spindle drive pulley to pull down on the idle drive belt; expecting the servo to sense the slight movement and respond by bringing in back to its original idle position. The employee stated that he had observed the previous maintenance manager test the servo this way as well as the out serviced technical support personnel, and had preformed the same test in the presence of the previous maintenance manager with the expected outcome. Rather than returning to idle as it had during previous tests, the servo started rapidly, pulling the employee's left hand into the spindle drive pulley severely cutting his second and third fingers. Although he was not hospitalized for greater than 24 hours, the employee stated that there was some bone loss to the tip of his left middle finger.
