Investigation Summary
On December 15, 2006, Employee #1 was working at a packing line in the facility. The packing line consisted of a chain conveyor with blowers and a belt conveyor several inches lower than the chain conveyor. With the crew preparing to take their break, the blowers and belt conveyor were shut off. Employee #1, whose work station was adjacent to the junction of the two conveyors, saw that several pouches of prunes had been placed on the chain conveyor. Employee #1 wanted to move them to the belt conveyor, and she reached underneath a metal hood with her left hand. Employee #1's hand became entangled between the operational chain conveyor and a small roller. Employee #1 was able to reach an emergency-stop switch. The small roller was removed to free Employee #1's hand. Employee #1 fractured one finger and suffered other trauma to additional fingers on the hand. Employee #1 was hospitalized for surgery but was released from the hospital later that evening.
