Investigation Summary

Investigation Nr: 202484325
Event: 09/08/2006
Employee Injures Hand While Operating Welder/conveyor System

On September 8, 2006, Employee #1 was operating an Amerimax welder/conveyor system that assembles vinyl window frames. As the machine completed welding a vinyl window frame together, the window became jammed at the junction connecting the welder to the conveyor. Without deenergizing the welder/conveyor system, Employee #1 walked around to the welder/conveyor junction and unjammed the window. However, when he turned to return to his original position, the glove on his left hand became caught in a revolving shaft of the conveyor. His hand was pulled into the conveyor, fracturing his left arm and wrist. A coworker, who heard Employee #1 yell for help, activated the emergency stop of the machine. Employee #1 was taken to Woodland Memorial Hospital, where he was hospitalized and underwent surgery for his injuries.
