Employee Is Killed in Fall Down Shaft

On August 25, 2006, a crew of four workers was in the process of moving a platform scaffold that was designed and engineered to fit inside a shaft within a mid-rise or high-rise structure. This platform was designed so that workers can safely install forming, pour concrete and remove the forming. Once the process is complete, the platform is raised to the next level. A crane lifts the platform and workers secure it. Employee #1 was securing the rigging to the platform so that it could be raised. He was not wearing any fall protection. He secured the west side of the platform and was working on the east side. The platform jerked out of the shaft, and Employee #1 was able to grab onto the rigging chain. The chain was swinging, and he struck the east wall, lost consciousness and fell approximately 200 ft down the shaft and died.
