Event: 10/16/2006
Employee Is Killed in Fall from Elevated Work Platform

On October 16, 2006, Employee #1 was working at the refrigerated warehousing and storage facility of a poultry processor. He was working from a cage attached to a Nissan forklift, with Serial Number 9C2206, when he fell from an height of approximately 28 to 30 feet. A witness to the accident stated that he saw Employee #1 come up over the side of the cage and that he heard a loud noise prior to the fall. Employee #1 sustained a head concussion. He was taken by Pope County emergency medical services to St. Mary's Hospital, where he died later that evening.
