Scaffold Overturns, Worker Later Dies

On October 6, 2006, an employee was taping windows, while standing on an extension ladder that he had placed on a 12-ft high scaffold that was planked with one 19 in. metal sheet. The employee climbed approximately 4 ft on the ladder, when the metal sheet slipped, causing the ladder and scaffold to overturn. The employee fell from the ladder and struck his head against several objects, as he fell to the concrete floor below. The employee was rushed to the nearest hospital, where he later died from severe head injuries.
