Employee's Arm Is Amputated in Machine

On August 24, 2006, Employee #1, a slitter operator, thought he was helping out by removing some debris from the winder cut-off slitter knife. This was not his regular job, but he had seen others do it. The area was surrounded by barrier guard fencing, but the access gate interlock was defeated and unbolted. The maintenance man had cleaned off the debris from the cut-off slitter, but never in between the roll stand and the frame. The jumbo roll stand was traveling back in position and the victim got his right arm caught between the moving roll stand and the frame. The right arm was amputated. Employee #1 was hospitalized.
