Employee Is Killed When Struck by Barstock in Head

At approximately 11:00 a.m. on July 27, 2006, Employee #1 was doing a bar change on a CNC machine. These bar changes occur every six minutes. The doors on the CNC machine were open, and the employee had his head inside the machine to line up the bar stock. His hand was on the manual control to move the chuck in and out, when he inadvertently turned the control the wrong way, thus sending the chuck into his head, causing multiple head fractures. Employee #1 was killed. The employee had been with the company for only four days. There were no safe work procedures in place, and the employee was left alone to operate the CNC machine. In the four days of his employment, he was trained by two trained CNC operators. Both of the operators told the employer that the employee was not qualified to operate a CNC machine. The employee apparently spoke very little English. The employer was going to fire him at the end of the day; however, he continued to let the employee operate two CNC machines.
