Employee Fractures Leg in Trip over Box

On February 14, 2009, an employee, a Technical Process Leader, was working in the Die Shop. Employees worked until 2:00 p.m. on Saturday. Shortly after 2:00 p.m., some of the lights were turned off and the rolls down doors were closed to the Die Shop. Between 2:15 p.m. to 2:30 p.m., the employee was walking back to his locker, located past a die oven, when he tripped and fell over a wooden box 24-in. long 24-in. wide by 18-in high that was located in front of the oven. The wooden box was dark in color and was not seen by the employee. The employee was hospitalized and treated for fractured leg.
