Investigation Summary
At approximately 11:00 a.m. on February 13, 2007, an employee was operating a mechanical power press bending small parts. The press did not have the point of operation guarded. The employee was attempting to remove the part when the machine cycled and caught his finger. The index finger of his left hand was amputated from the second knuckle up. The employee was not hospitalized. In addition to the point of operation not being guarded, other hazards were noted with the machine such as no guard on the foot pedal, no slip pad on the foot pedal, fly wheel not guarded, did not incorporate single stroke, no inspections of the equipment were being conducted.
