Employee Is Killed When Struck by Dislodged Clay

At approximately 8:55 a.m. on April 05, 2007, an employee was standing outside the confines of the 8-ft tall by 11-ft wide by 20-ft long trench box. The trench box was placed in a trench that measured 12-ft deep by 11-ft wide by 20-ft long. The employee was preparing to remove a plastic cover from a 4-in. plastic pipe that was connected to a 8-in. sewage line that was being installed. The employee was struck by a large piece of dirt or clay that dislodged from the trench wall in the area the employee was standing. This resulted in the employee breaking his neck. The employee was killed.
