Employee Is Killed When Struck by Falling Object

At approximately 9:00 a.m., on May 2, 2008, Employee #1, of Advanced Concrete Systems, was pouring concrete foundation walls using a concrete bucket attached to an overhead crane. Attached to the lower portion of the bucket was a vibrating screed used to finish the concrete. While in the process of moving the bucket/screed to another bed, a ring used to attach the bucket to the hook of the overhead crane broke. The bucket and screed fell to the bed. Employee #1 was standing on the south side of the bed when the bucket fell. It was believed that the screed struck him in the head and killed him.
