Employee Is Killed in Fall from Rafter

On September 9, 2006, Employee #1 was working from a work platform that was attached to a JLG rough terrain forklift, to tighten some bolts on a metal rafter that were approximately 30 feet above the ground. He had difficulty reaching the bolts and decided to jump to the rafter to get a better reach. While jumping to the rafter, Employee #1 fell to the ground. He was transported to North West Texas Hospital in Amarillo, TX, by helicopter, where he was pronounced dead. The accident investigation revealed that Employee #1 was wearing a body belt at the time of the accident, but was not attached to an anchor.
