Investigation Summary
On October 19, 2006, Employee #1 was dismantling freezer rooms. A coworker used a forklift to lift and position Employee #1 for removal of ceiling panels within a freezer room. Several of the ceiling panels fell causing Employee #1 to loose his balance. He fell, approximately 8 ft, forward from the wooden pallet and landed on several metal racks. Employee #1 was hospitalized and died.
