Employee Is Injured When He Falls from Roof

On October 19, 2006, Employee #1 was working on the second floor of a penthouse on the top of a new 22-story building. He was stripping forms from previously poured concrete adjacent to an unprotected edge, approximately 12.33 ft above a lower level. There was no guard rail, safety net, or personal fall arrest system in place. He fell receiving injuries to his hand, ribs, feet and was hospitalized.
