Employee Receives Multiple Fractures in Fall from Guardrail

On April 6, 2006, Employee #1 was cleaning concrete spoils off the existing slab from columns that had been formed and poured on the fourth floor of Building Number 4 at the Artesia construction site. While scraping the concrete with a shovel, he lost his balance, causing him to lean on the guardrail system. The guardrail system failed, which resulted in his fall from the fourth floor to a second floor. He fell approximately 24 ft, striking a second floor concrete slab pn the way down and sustaining multiple fractures across his body. Emergency medical personnel were summoned. They arrived and transported him to a medical center. At the hospital, he underwent surgical procedures to repair the multiple fractures. After undergoing the surgical procedures, he was hospitalized for postoperative treatment. The investigaton revealed that the guardrail had been altered from the original installation to place the column forms and had not been sufficiently fastened to the column forms once they had been installed.
