Investigation Summary
On August 21, 2006, Employee #1 was coring a hole for a future piping system. After raising the bit and locking it in place, he began to clear the hole. When the locking mechanism failed, the coring bit descended and struck Employee #1's fingers. He was hospitalized for finger amputations.
