Investigation Summary
At approximately 7:30 a.m. on June 13, 2008, Employee #1 was standing on a plank placed on unsupported and improperly installed side brackets of a metal scaffold. The scaffold was not used according to the manufacturer's recommendations. The side bracket was not installed and or seated correctly. Employee #1 fell 12 ft receiving body fractures, not specified, and was hospitalized.
