Investigation Summary
At approximately 12:45 p.m. on April 28, 2008, Employee #1 was operating a web cutting machine. The web ran out and he was removing the web with his hand while jogging it out with his other hand. Employee #1 was injured when he pushed the wrong button and the knife and partially amputated his right thumb and index finger. He was not hospitalized.
