Investigation Summary

Investigation Nr: 202466728
Event: 05/06/2008
Employee Catches Finger Between Two Rollers

A machine operator was feeding a 3.7-meter-long, 14-millimeter-diameter aluminum bar into a roll-marking machine. A forklift supported the bar at one end, and the machine operator was using his left hand to guide the other end into the feed channel. As his hand approached the rollers, his glove caught in the feeding channel and pulled his left index finger between the two roller dies. The employee suffered an amputation of his left index finger to the second joint. He had reconstructive surgery and was hospitalized for 3 days.
