Investigation Summary

Investigation Nr: 202465936
Event: 04/01/2008
Employee's Back Is Fractured Crushed Under Falling Object

At approximately 1:55 p.m. on April 1, 2008, Employee #1, a journeyman, and a coworker, a welder, were assigned to remove a tank, motor and other items from the trailer and to fabricate and install a barbeque box on the trailer. The trailer was manufactured by Alba Manufacturing, trailer Model Number 2A610S, Serial Number CA80AMG1019. Employee #1 attached the hook from a jib crane to the passenger side of the 6 ft by 10 ft tandem axle trailer. Once the hook was attached to the bottom edge of the trailer near the middle of the tandem axles, he engaged the lift. The jib crane lifted the passenger side approximately 5 ft. Employee #1 then positioned himself under the trailer to use a cutting torch to burn the welds free from the diamond plate that was welded to the trailer frame. While Employee #1 was cutting the welds free the hook became disengaged allowing the trailer to fall on top of him. Employee #1 received compression fractures to his back and was hospitalized.
