Investigation Summary
At approximately 4:18 p.m. on February 28, 2008, Employee #1 was working for beverage can manufacturer. When he reached into the 12 oz Body Maker N:16 to remove a can from the can pocket, his right index finger was amputated by an unguarded rotary cup infeed drive chain and sprocket. Employee #1 was hospitalized. The investigation concluded that violation of CCR Title 8 Section 4075(a) was the cause of this accident. As a result, the Division issued serious accident related citation to the employer according CCR Title 8 Section 4075(a).
