Investigation Summary
At 3:20 p.m. on December 18, 2007 , Employee #1, was performing a regularly assigned task of set up procedures in an attempt to set up a new vertical milling machine. Employee #1 was assisting a coworker to set up a newly purchased machine by a client. Both Employee #1 and his coworker were at the site of clients' building where the packaging material had been removed. Prior to incident employee #1 was pointing his finger at the tool changer arm which rotated counterclockwise parallel to the floor of the machine. The coworker was at the control panel initiating start up procedures and tolerances for parts. Employee #1 put his hand in close proximity to the tool changer arm to point out to hs coworker, there was an unwanted variation in the movement. The coworker hit some buttons at same time Employee #1 had his finger within the swing zone of arm. The arm moved very fast and severed the upper portion of his right index finger. The injured employee was working within close proximity to his coworker who was operating the controls. Employee #1 was transported to the emergency room at Henry Mayo Hospital in Valencia where his finger was amputated. The investigation concluded that Employee #1 did not receive any type of training on control of hazardous energy, and was considerably involved in setting-up and adjusting the milling machine without any training.
