Investigation Summary

Investigation Nr: 202465852
Event: 11/14/2007
Employee's Finger Is Amputated in Unguarded Machine

At approximately 1:44 p.m. on November 14, 2007, Employee #1, a machine operator, was placing stainless steel bearings on a magnetized table to grind the tops down to a specific size. He placed bearings on the magnetized table for surfacing then pressed a button to turn off the upper rotating wheel with the diamond tips used to grind the top surface. As the upper wheel was slowing down, he placed his left hand inside the work area while wearing gloves. There was a slight gap between the two points of contact, and there was no guarding of the hazardous moving parts. His hand was on top of the rotating table and the tip of his left hand glove was caught between the rotating wheels. His hand was pulled in with the glove, between the two points, amputating his finger. Employee #1 was hospitalized.
